Job Summary
We are seeking a passionate and dedicated Wellness Coordinator to join our assisted living community. Duties
- Develop, coordinate, and oversee personalized care plans that address residents’ unique health needs, including Alzheimer’s and dementia care.
- Supervise and support staff members involved in resident care, ensuring adherence to safety protocols, medication administration, and quality standards.
- Collaborate with nursing staff, social workers, and family members to monitor residents’ progress and adjust care strategies accordingly.
- Lead wellness activities and programs designed to promote physical activity, mental stimulation, social engagement, and overall well-being.
- Manage operations related to health services within the community, including budgeting, resource allocation, and compliance with regulatory requirements.
- Conduct regular assessments of residents’ health status and coordinate necessary interventions or referrals to healthcare providers.
- Maintain detailed documentation on resident care plans, incident reports, medication logs, and other essential records with accuracy and confidentiality.
Experience
- Proven experience working in assisted living facilities or nursing homes with a strong background in senior care or long-term care environments.
- Demonstrated supervisory or management experience overseeing caregiving teams or group home staff.
- Extensive knowledge of Alzheimer’s disease, dementia care practices, memory care strategies, and resident safety protocols.
- Familiarity with developing and managing care plans tailored for individuals with developmental disabilities or requiring specialized support.
- Experience in operations management within healthcare settings including budgeting, resource planning, and compliance oversight.
- Background in social work or related fields that emphasize resident-centered approaches to wellness and community engagement.
- Prior home & community care experience is highly desirable for understanding diverse resident needs across different settings.
