Overview
We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including retirement homes, nursing homes, hospitals, assisted living facilities, and home care environments. This role offers an opportunity to work with diverse patient populations, including seniors, children, and individuals requiring specialized care such as dementia, hospice, or acute care. The RPN will collaborate with multidisciplinary teams to ensure optimal health outcomes and uphold the highest standards of nursing practice.
Responsibilities
- Must have the ability to speak other languages and able to be culturally sensitive
- Deliver comprehensive patient care in accordance with physician orders and established protocols
- Administer medications and treatments accurately and safely, including medication management and pain control
- Monitor patient vital signs and assess health status regularly
- Assist with procedures such as phlebotomy and laboratory specimen collection
- Maintain detailed documentation using electronic health records systems like YARDI, Epic, Cerner , eMAR
- Support patients with activities of daily living and provide emotional support
- Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans
- Educate patients and families about health conditions, medications, and post-discharge care instructions
- Ensure compliance with infection control standards and safety protocols in all patient interactions
- Participate in case management activities to coordinate ongoing care needs
Qualifications
- Current registration as a Registered Practical Nurse (RPN) with the appropriate licensing body
- Proven experience in nursing home, hospital, or acute care settings preferred
- Knowledge of medical terminology, medication administration, and patient care procedures
- Experience with specialized areas such as dementia care, pediatrics, hospice, senior care, or home care is highly desirable
- Familiarity with electronic health record systems such as Yardi, Epic or Cerner is an asset
- Strong communication skills and ability to work effectively within a multidisciplinary team
- Ability to handle multiple priorities in a fast-paced environment while maintaining attention to detail
- Compassionate approach to patient-centered care with a focus on dignity and respect
