* Registered Practical Nursing

* candidates for a permanent full-time

* DAY* rotation AND a full-time

* NIGHT* rotation in addition to permanent part-time opportunities.

* Duties/

Responsibilities:

* Assists Unit Manager to:

* Complete and ensure resident assessments are accurate, up to date and changes communicated as required

* Plan, coordinate, manage and evaluate the resident’s care plan working with the resident/substitute decision maker, physician, nursing and

interdisciplinary

team and contracted service providers

* Under the direction of the Medical Director and/or Nurse Practitioner, process and follow orders.

* Assess and accurately reports resident response to care in a timely manner

* Prepare for scheduled care conferences and participates in the conferences as applicable

* Responds to resident/family concerns and ensures appropriate action is taken and/or concerns communicated to the Unit Manager and/or Director of Care

* Responsible for the safe and accurate administration of medications and treatments. Additionally, to assess and document response to treatment and report any incidents related to medication administration to the Unit Manager and Medical Director

* Provides

education

and support to residents and families by maintaining an open, knowledgeable rapport. Communicates changes in resident’s condition as necessary, in an ethical manner which follows privacy and confidentiality protocols

* Responds to and manages significant changes in resident’s condition and or medical emergencies; provides emergency first aid

* Provides supervision and direction to PSWs while promoting a

multidisciplinary

team approach to care and service

* Assists to evaluate the performance of PSWs annually and as needed

Requirements:

* * Current registration as Registered Practical Nurse in the province of Ontario

* Experience working in a long term care environment and with unregulated care providers preferred

* Strong written and oral

communication

, care coordination, time management and customer focus skills

* Demonstrated specific knowledge /skill in current geriatric nursing care and standards preferred
