GENERAL SUMMARY:

Under the general direction of the Operations Manager, Executive Health, the Referral Coordinator performs the following functions:

Coordinates patient referral appointments to outside specialists and in-house clinicians.
Works with physicians, nurses, program advisors and service coordinators to oversee the follow-up process for all Executive Health clients and other CCC patients.
Coverage for Switchboard and Medical Records department
Assists with development, improvement and evaluation of Executive Health referral procedures related to customer service and quality improvement.
Committed to providing proactive individualized customer service for all patients.
Actively participates in staff meetings.
Excellent oral and written communication.
PC literate, including Microsoft Office products.
Ability to work in a team environment
Ability to meet or exceed Performance Competencies
Ability to work under pressure and be adaptable to changing procedures and priorities while remaining flexible and responsive to requests
Must have a proven work record of dependability
Candidate must have the ability to handle multiple tasks in a fast paced medical organization.

MINIMUM QUALIFICATIONS:

Education, Knowledge, Skills and Abilities –

Strong interpersonal skills, verbal and written communication skills are needed as the role requires frequent communication to patients, outside physicians and their office staff, as well as internal staff at all levels of the organization.
Significant attention to detail and organizational skills are required.
Experience working with EPIC or other electronic medical record system is an asset.
Strong problem-solving skills required to coordinate multiple resources and requests in a short time frame.
Expert knowledge of medical terminology attained through experience or formal coursework is required.
Ability to multi-task in a fast paced environment is critical to success.

PRINCIPLE DUTIES AND RESPONSIBILITIES:

Coordinates all referral appointments for patients (both inside and outside facility).
Maintains and updates referral source database, provides all appointment instructions and information to clients, determines urgency of appointments and prioritizes as needed.
Meet with patients in person or by phone to summarize follow-up appointment requests (ie. testing, referrals, wellness appointments, etc.).
Acts as a liaison for patients as needed.
Assists with development, improvement and evaluation of referral procedures related to service and quality improvement measures.
Provides timely follow-up to upset patients and alerts manager to work together to find solutions to problems.
Documents all patient correspondence in electronic medical record (EMR). Responsible for obtaining and test results from outside providers and to ensure they are reviewed by physician and scanned into EMR.
Coverage for Patient Services Representatives when required
Other duties as required.
