About the Role

We are a growing pulmonary practice providing specialized respiratory care across skilled nursing and rehabilitation facilities. We are seeking a motivated and clinically strong Nurse Practitioner to join our team.

Position Overview

  • Provide pulmonary-focused care in skilled nursing and rehabilitation facilities
  • Manage patients with COPD, asthma, chronic respiratory failure, and complex pulmonary conditions
  • Comfortable managing tracheostomy and ventilator-dependent patients (or willingness to learn)
  • Perform high-quality follow-up visits and new evaluations
  • Typical volume: 15–20 patients per day

Compensation Structure

  • Initial per diem / per-visit compensation model
  • Structured to reward both volume and high-quality, billable documentation
  • Strong earning potential based on productivity (typically equivalent to $90k–$130k+ depending on schedule and volume)

As credentialing expands and patient volume grows, there is a clear pathway to:

  • Increased clinical days
  • Transition to a more advanced compensation model tied to collections
  • Opportunity for significantly higher earnings

Schedule

  • Flexible schedule, starting 2–3 days per week
  • Opportunity to expand to additional days or full-time role as practice grows

Responsibilities

  • Evaluate and manage pulmonary patients in SNF/rehab settings
  • Manage oxygen therapy, inhaler regimens, and respiratory treatments
  • Care for patients with tracheostomies and ventilator needs
  • Communicate effectively with nursing staff and facility teams
  • Coordinate care with primary providers and consultants
  • Complete timely, detailed, and billable clinical notes

Qualifications

  • Nurse Practitioner licensed in New York
  • Strong clinical judgment and ability to work independently
  • SNF, inpatient, or pulmonary experience preferred
  • Comfort with high-volume clinical workflow
  • Experience with tracheostomy or ventilator management preferred (or willingness to learn)
