We are currently seeking a:
Program Assistant—Responsive Community Care (HMNS)
Full-Time Contract ending December 31, 2024 (Hybrid Model)
8:30am-4:30pm (Mon-Fri)
What is the job?
To provide program support and coordination to all programs operating in Community Support Program unit such as administrative, planning and scheduling assistance during absences or high-volume situations. To perform all intake functions: receive, assess, and process all requests for service from potential clients and referral sources, provide referral and information regarding the criteria for service, the range of services offered and referral services to other community services, provide and participate in outreach activates. Participate in VHA’s services and quality initiatives. All activities are conducted within the policies, procedures, and standards of confidentiality of VHA.

Key Skills, Experience and Behaviours Required for this Position:
Social Service Worker Diploma or relevant degree combined with a minimum of 2 years’ experience
Knowledge of Community resources and Community Health Care Sector
Knowledge of Mental Health and Addictions
Excellent problem-solving skills, solution-focus approach and crisis intervention
Ability to handle a high volume of telephone intake calls and inquiries
Excellent telephone skills—the ability to interact professionally with callers
Knowledge of Microsoft Office and the use of scheduling programs
Excellent keyboarding skills and accurate data entry skills
Speed and accuracy in computer inputting (50 wpm)
Excellent verbal, written and interpersonal communication skills
Demonstrate efforts to continuously develop and upgrade skills
Strong organizational skills with attention to detail and the ability to prioritize
Knowledge of the city (GTA) and TTC routes
A valid Ontario Driver’s License and the use of a car an asset
Satisfactory completion of technical skills testing is required (typing, MS …
