Responsibilities:
- Evaluate and treat patients in the home setting
- Build individualized, goal-oriented plans of care
- Deliver therapeutic interventions and functional training
- Educate patients and caregivers
- Complete timely EMR documentation
Requirements:
- Active NJ PT license (new grads eligible)
- Degree from an accredited PT program
- Reliable transportation
- Strong communication and time-management skills
