Position: Physical Therapist (Home Therapy)
Location: Northern Indiana (Elkhart and Goshen area)
Schedule: Flexible, therapist‑driven
Employment Type: Part‑time or full‑time opportunities available
⭐ What You’ll Do
- Provide skilled physical therapy in the patient’s home environment
- Complete evaluations, 30‑day reassessments, and recertification visits
- Develop individualized treatment plans focused on safety, mobility, and functional independence
- Collaborate closely with our OT, ST, and administrative team
- Educate patients and caregivers on home exercise programs and fall‑prevention strategies
- Document visits accurately and efficiently in our EMR

- Licensed Physical Therapist (PT) in the state of Indiana
- Strong communication and clinical reasoning skills
- Ability to work independently in the home setting
- Reliable transportation
- Home health experience preferred, but we’re happy to train the right PT
