The Peer Senior Companion provides person-centered peer support, companionship, advocacy, and resource navigation to older adults experiencing social isolation, loneliness, life transitions, chronic health conditions, grief, or other aging-related challenges. Drawing from lived experience and peer support principles, the Peer Senior Companion fosters meaningful connections that enhance social engagement, emotional wellness, and independence. The Peer Senior Companion works directly with participants in homes, nursing facilities, community settings, and MHAOC locations, facilitating individual and group activities while connecting seniors to community resources and support services.
RESPONSIBILITIES INCLUDE:
» Maintain an active caseload and provide person-centered peer support, companionship, wellness check-ins, and resource navigation to older adults experiencing social isolation, health challenges, or life transitions.
» Conduct telephone calls, home visits, facility visits, and community-based outreach to engage isolated and underserved seniors throughout Ocean County.
» Facilitate social, recreational, educational, and wellness activities that promote connection, independence, and emotional well-being, including group discussions, community outings, workshops, and intergenerational programs.
» Develop and maintain collaborative relationships with senior centers, healthcare providers, nursing facilities, community organizations, and other stakeholders to enhance service accessibility and participant engagement.
» Advocate for participant needs, rights, preferences, and inclusion while promoting successful aging in place and improved quality of life.
» Maintain accurate and timely documentation, administer program assessments, track outcomes, and ensure compliance with agency policies, confidentiality standards, and funding requirements.
» Participate in staff meetings, trainings, community events, and other duties as assigned in support of the Senior Community Companion Program.
» Other duties as assigned by supervisor.
QUALIFICATIONS:
» Valid driver’s license with a clean driving record.
» High school diploma or equivalent required
» Lived experience as an older adult navigating aging-related challenges, life transitions, caregiving experiences, chronic health conditions, recovery, or social isolation preferred.
» Experience working with older adults, peer support programs, community outreach, behavioral health, or senior services preferred.
» Knowledge of aging services, Medicare, Medicaid, community resources, and social determinants of health.
» Understanding of person-centered, trauma-informed, and recovery-oriented practices.
» Strong interpersonal communication, active listening, and problem-solving skills.
» Ability to establish rapport and maintain professional boundaries.
» Ability to work independently and collaboratively within a multidisciplinary team.
» Proficiency in basic computer applications and electronic documentation systems.
» Reliable transportation and ability to travel throughout Ocean County.
» Bilingual or multilingual abilities preferred.
Pay: $18.00 – $22.00 per hour
