Position: Occupational Therapist (Home Therapy)
Location: Northern Indiana (Mishawaka/South Bend area)
Schedule: Flexible, therapist‑driven
Employment Type: Part‑time or full‑time opportunities available
⭐ What You’ll Do
- Provide skilled occupational therapy in the patient’s home
- Complete evaluations, 30‑day reassessments, and recertification visits
- Develop individualized treatment plans focused on ADLs, safety, cognition, and functional independence
- Collaborate closely with PT, ST, nursing, and caregivers
- Educate patients and families on adaptive strategies, equipment use, and home safety
- Document visits accurately and efficiently in our EMR

- Licensed Occupational Therapist (OT) in the state of Indiana
- Strong communication and clinical reasoning skills
- Ability to work independently in the home setting
- Reliable transportation
- Home health experience preferred, but we’re happy to train the right OT
