VON is currently seeking Occupational Therapists to join the VON team!
Occupational Therapists will be supporting two
innovative
programs, CAPABLE and the Employee Support Program.
The Community Aging in Place, Advancing Better Living for Elders (or CAPABLE) program aims to enhance the functional mobility, independence, and safety of economically disadvantaged adults aged 65 and above who remain independent and in their own homes. Online training and support are provided to become a certified CAPABLE clinician.
Our Employee Support Program provides workplace
safety training
to nurses, CCAs, and other care workers who deliver home care services. The Occupational Therapist collaborates with home care organizations, their occupational health and safety teams, and employees to assess needs, address concerns, and develop recommendations to prevent injuries and ensure smooth return-to-work transitions.
Key Responsibilities:
Demonstrates knowledge, skills and attitudes as outlined in the Occupational Therapy standards and guidelines for
evidence-based practice
in order to assess, plan, and evaluate progress (for potential discharge) and document a patient plan of care within a patient-centred approach to therapy goals
Assesses the client’s ability to function safely in the home/community environment
Assesses clients from a
holistic approach
, i.e. mental health status, social contacts, and spiritual and physical needs that may potentially impact the patient’s ability to maintain independence in a home environment
Develop a client-centred plan with the client and healthcare team Provides
education
and recommendations to clients pertaining to the therapy goals
Monitors client outcomes and progression toward therapy goals
Modifies client’s care plans as required based on patient outcomes
Documents client outcomes and current care plans in the electronic patient chart according to the funder and College of Occupational Therapists of Nova Scotia guidelines
Completes and updates the written care plan (electronic copy) with information collected from the patient, his/her family and any other healthcare or educational professionals involved
Demonstrates professional code of conduct/
communication
with all members of the health team as per VON guidelines and policy
Education
, Designations and
Experience:
Masters in Occupational Therapy (or degree prepared occupational therapy
education
prior to 2008)
Valid registration with COTNS and associated liability insurance
Membership with the Nova Scotia Society of Occupational Therapists is preferred
