We’re seeking a compassionate and motivated Licensed Social Worker to join a skilled nursing team in Austin, TX. This is an opportunity for a Licensed Social Worker who is passionate about supporting residents and families through care transitions, psychosocial support, and advocacy in a fast-paced healthcare environment.
What You’ll Do:
- Provide psychosocial assessments for residents and develop individualized care plans
- Support residents and families through adjustment to illness, long-term care placement, and end-of-life planning
- Coordinate discharge planning and safe transitions of care in collaboration with the interdisciplinary team
- Serve as a key advocate for resident rights, needs, and quality of life
- Provide counseling, emotional support, and resource navigation for residents and families
- Maintain accurate and timely documentation in compliance with regulatory standards
- Collaborate closely with nursing, therapy, and administration to ensure holistic resident care
What We’re Looking For:
- Active Licensed Social Worker (LSW, LMSW, or LCSW) in Texas (or eligible for licensure)
- Experience in skilled nursing, long-term care, hospice, or healthcare setting preferred
- Strong communication, assessment, and crisis intervention skills
- Ability to work effectively in a multidisciplinary team environment
- Passion for advocacy, resident support, and improving quality of life
