3PM-11:30 PM LPN Supervisor
80 Hour Bi-weekly position; E.O.W. ; E.O.H.
The Meadows Manor is seeking an experienced LPN in LTC/PCF setting- willing to train the right applicant!
Willingness to lead a dedicated team, provide, instruction, implement and develop policies, participate in employee improvement actions. Must be willing to flex schedule during extenuating circumstances.
General Responsibilities
Responsible for developing and maintaining a personal care services care program that is compatible with the philosophy of The Manor; insuring the department’s compliance with operating policies and procedures as well as outside regulatory requirements; organizing the work and activities of the department; communicating with appropriate personnel; directly supervising assigned personnel; providing periodic reports; coordinating the personal care program with other departments of the home so as to enhance the concept of total gerontological care; coordinating annual surveys in conjunction with the Department of Public Welfare.
Essential Duties and Responsibilities
1. Trains, supervises, motivates, and evaluates staff to ensure the provision of high quality services to residents in a manner that is consistent with the philosophy of assisted living. May be responsible for teaching the Medication Training Course required by DPW for new Resident Assistants. Works with Resident Care Manager in orienting new employees. Schedules employees when the Resident Care Manager is not working or when on call.
2. Plans and delegates work assignments equally and as indicated in the Resident Assistant job description. Monitors resident laundry; assuring that each resident’s laundry is returned to them within 24 hours.
3. Coordinates move-in and orients new residents to the facility, ensuring satisfactory integration into the facility/community.
4. Monitors resident service plans, records information to determine quality of service to residents, and directs staff to implement needed change.
5. Reports significant changes in a resident’s condition to his or her physician at all times. Keeps Resident Care Manager informed of any change in the resident’s condition.
6. Documents medication supervised and notes resident’s response. Follows through on verbal medication change orders received from the physician–including physician signature within 48 hours and writing order on the MAR.
7. Documents resident incidents. Checks on resident after incident reports are filled out as directed by the Resident Care Manger.
8. Demonstrates the ability to function as a team member in sharing responsibilities for the administration of resident services in cooperation with all members of the team.
9. Does quarterly quality management with Resident Care Manager. This includes correcting any errors found and documenting that the errors are corrected.
10. Monitors ordering of medications and checks for expiration dates for medications.
Checks in medications on the 3-11:30 shift.
11. Subject to 24 Hours a day on call status every 3rd week alternating with the Resident Care Manager and Resident Assistant Supervisor. On call means accepting full responsibility and all decisions made.
12. Provides ongoing training of the medication training program by DPW.
13. Promotes and facilitates interactive problem solving with the Resident Care Manager.
14. Coordinates move-out, ensuring proper labeling and packing of resident’s belongings, if necessary.
15. Regularly inspects personal care area and personal care services to insure continuing compliance with federal, state, and local standards and regulations.
16. Insures that preventive measures, medications, treatments, and other health services are properly prescribed, carried out, and recorded. Checks that all prescribed medication is available to residents – assuring the medication is ordered in a timely manner to prevent any charges to the facility.
17. Insures that all necessary documentation is being accomplished, i.e., reviews the documentation of personal care services personnel to insure it is correct and complete, receives input regarding quality assurance; assisted in revising, designing, and choosing forms used in the documentation process.
18. Reports problems to the Resident Care Manager and the Administrator.
19. Works and communicates with families, physicians, and pharmacists.Coordinates doctor’s appointments with the main office. Prepares envelope with necessary documents and addresses any orders that are a result of the doctor appointment. Ensures that all lab work is done as ordered, and the physician informed of the results and the physician’s response documented in conjunction with the Resident Care Manager.
20. Meets with residents regarding medical issues and complaints; answers questions and solves problems; investigates and responds to complaints initiated by residents, staff, or family members.
21. Knows the Department of Public Welfare regulations for the facility and adheres to the regulations.
22. Insures the department and all personnel adhere to established operating policies and procedures along with outside regulatory requirement.
23. Responsible for following thru with medication error reporting to the Resident Care Manager.
24. Follows individual assigned procedures in cases of emergency according to the Fire and Safety Manual and Policies & Procedures Manual.
25. Attends in-service and other related meetings as required.
26. Performs other related duties as assigned or directed.
27. Must possess strong organizational and time-management skills.
28. Maintain employee, facility and resident confidentiality at all times.
29. Responsible for duties of the Resident Care Manager when the Resident Care Manager is on vacation or sick or leaves early.
30. Works on floor with resident assistants; working as a resident assistant as needed. Responsible for supervising work of Resident Assistants on the floors.
Required Education, Training, and Experience
A graduate of an approved school of nursing; current license to practice nursing in Pennsylvania as a Licensed Practical Nurse; nursing care; current CPR/First Aid certification (or eligibility and pursuit of certification); proficient interpersonal relations and communicative skills; demonstrated management and supervisory skills; minimum one (1) year of experience in related positions. Must pass Direct Care Staff Person Training Course and the Medication Training Course
