Responsibilities
- Develop, coordinate, and update individualized plans of care, ensuring continuity and collaboration with the interdisciplinary team (IDG) and other staff.
- Conduct initial and ongoing comprehensive assessments, including physical exams, medical history reviews, and environmental evaluations to determine patient and family needs.
- Provide nursing services, treatments, and preventive procedures requiring specialized skills as ordered by the physician.
- Monitor and document changes in patient condition, pain/symptom management, and reactions to treatments, ensuring timely communication with the care team and physicians.
- Educate patients and families on care plans, hospice services, and health-related topics to empower informed decision-making.
- Supervise and evaluate the care provided by Hospice Aides and Licensed Practical Nurses (LPNs), ensuring compliance with care plans and conducting regular supervisory visits.
- Coordinate referrals to other hospice care disciplines and document rationale if referrals are not made.
- Perform on-call responsibilities to address patient and family needs during assigned times, including emergency care.
- Maintain accurate, timely, and detailed patient records in compliance with agency policies and regulatory standards.
- Participate in staff development meetings and contribute to the ongoing improvement of care delivery and patient outcomes.

Qualifications:
- Graduate from an accredited School of Nursing.
- Current licensure as a Registered Nurse (RN) in the state.
- Strong assessment and problem-solving skills.
- Proficient in organization, time management, and understanding case management processes.
- Valid driver’s license with access to a reliable, insured vehicle in good working condition.
- Excellent verbal and written communication skills.
- Clear RN license with no restrictions; must be able to administer medications, perform medication reconciliations, and provide nursing procedures or treatments within the scope of state nursing practice.
- Completion of competency assessments prior to patient care and as needed for less common procedures or treatments.

Job Type: Full-time
Pay: $75,000.00 – $85,000.00 per year
