Position SummaryThe Health Promoter will function as part of an interdisciplinary collaborative team within the NorWest primary care team and chronic disease team at NorWest Co-op Community Health and will support community-based health promotion programs including individual nutrition assessment, program planning and community outreach.
The Position Will Be Responsible For
Providing individual nutrition counseling services to clients of all ages as well as group education and health promotion activities within the Inkster/Seven Oaks community area.
Supporting health promotion initiatives (hans kai, health management group classes, community requests) as required
Maintain accurate records of project activities, statistics and other program deliverables
Working collaboratively with the chronic disease team supports and the Inkster/Seven Oaks My Health Team.
Qualifications
Registered Dietitian required, member in good standing with the College of Dietitians of Manitoba, and possesses professional liability insurance
Minimum two years recent experience working in community based and/or primary care setting.
Excellent knowledge of the field of health promotion with the ability to plan, implement, facilitate and evaluate programs.
Knowledge and experience working with diverse cultural groups.
Experience in group facilitation.
Able to identify nutrition problems, assess nutritional status of patients, develop nutrition care plans and evaluate and monitor effectiveness of nutrition interventions.
Ability to offer 1-1 and group education supports around chronic disease management (diabetes, hypertension, heart disease, etc.)
Ability to work independently as well as collaboratively within an inter-professional team
Ability to work flexible hours, including occasional evenings/weekends as required
Extensive knowledge of community resources.
Excellent interpersonal and communication skills required.
Competent in use of Accuro EMR (Electronic Medical Record)
Valid Driver’s License as well as access to a vehicle.
Position to commence as soon as possible.
