Deliver patient-centered care as a Field-based Licensed Vocational Nurse. Provide skilled nursing visits, assessments, and education while collaborating with the clinical team to ensure safety and care continuity.
In this LVN role, you will deliver hands-on nursing care within LVN scope and under RN supervision. Focus on patient assessments, medication safety, and wound care tasks. Strong communication skills are essential to escalate patient conditions and document effectively in Homecare Homebase.
Key Responsibilities:
• Perform skilled nursing visits in home settings
• Conduct patient assessments and monitor conditions
• Educate caregivers and patients on care plans
• Collaborate with the interdisciplinary team
• Document care accurately and promptly in HCHB
Requirements:
• Current Texas LVN license required
• Comfortable with field-based workflows
• Strong skills in communication and documentation
• Reliable in following care orders
• Current CPR certification (BLS) mandatory
