Become an integral part of the Sunnybrook Academic Family Health Team as a Registered Nurse, providing outpatient care and health education. Focus on promoting health, preventing disease, and managing chronic conditions.
This position demands primary health care experience and the delivery of nursing services to diverse patient populations, including high-risk groups. You will assess health needs, administer treatments, and educate patients on health resources and personal health decisions while working collaboratively within an interdisciplinary team.
Key Responsibilities:
• Assess patient health care needs for appropriate interventions
• Triage patient calls for effective scheduling
• Analyze health assessment findings and interpret records
• Monitor and report changes in patient conditions
• Administer prescribed medications and vaccinations
Requirements:
• Registration in good standing with the Ontario College of Nurses
• Experience in outpatient clinics and primary health care
• Strong clinical and communication skills
• Ability to adapt in a dynamic environment
• Proficient with EMRs, especially Telus Practice Solutions
