Job Summary

Job Description
Major Responsibilities:

Processes applications and referrals from residents.

Conducts assessments of residents’ needs and eligibility for service and assistance.

Obtains required information from residents and next-of-kin and consults and collaborates with doctors and other team members, agencies, etc., to complete appropriate documentation.

Provides crisis intervention during emergency and other situations.

Determines, develops and recommends course of action based on legislation, divisional policies, procedures, functional abilities, urgency, finances, etc.

Counsels the individual and/or families on best alternatives or course of action.

Assists or coordinates referrals, provides adjustment and support to new situations through additional counselling, observation and evaluation.

Provides therapeutic counselling and ongoing support to residents/families regarding behaviour, attitude, and environment and advocates on their behalf.

Refers individuals and/or residents to internal or external programs, services and agencies to complement, or in place of City services.

Coordinates referrals for admission to outside agencies, purchased services and City services by preparing forms, writing reports, presenting residents’ cases to admission boards and providing involved staff with information necessary to prepare for admission (e.g. ensuring accommodations are prepared etc.).

Monitors individual and/or families’ progress by additional visits, counselling, discussions about change in status, needs, etc.

Liaises with agencies involved regarding residents’ attendance, suitability, concerns, etc. and modifies action plan, as required.

Writes reports, prepares correspondence and maintains case documentation. Documents all information, occurrences, etc., and prepares statistics related to case histories. Maintains records, charts, clinical reports, files, waiting list, etc.

Participates in care conference meetings to review individuals’ and/or families’ goals, outstanding needs, program suitability, develop resident care plans and identify expected outcomes. Develops and conducts group sessions.

Develops objectives and appropriate evaluations of the impact of each session on the resident’s wellbeing, status, etc.
