Contract Registered Social Worker

Blue Sky Family Health Team is currently seeking two (2) short term (ending Mar31/23) contract
Registered Social Worker (RSW) to join our interdisciplinary team of health care providers. The RSW will work collaboratively with all members of our team by providing comprehensive primary health care in consultation with our team of family physicians.

Contract Registered Social Worker

Candidates must have expertise and comfort working with complex mental health problems. The primary focus of practice includes: individual and couple counselling, psychosocial therapeutic functions and psychotherapy for anxiety, depression, and trauma. The RSW should be knowledgeable of community resources in the areas of mental health and crisis services to ensure a seamless transition to community resources, if appropriate.

Qualifications:

• Master’s Degree in Social Work
• Minimum of 3 years’ experience
• Registered with the Ontario College of Social Workers and Social Service Workers
• Previous related patient care experience, specifically in the primary care setting preferred
• Experience with Electronic Medical Records an asset
• Exceptional oral, written, and presentation skills; including strong interpersonal skills
• In depth knowledge of chronic illness care, including both clinical and support care
• In depth knowledge of the local health program delivery and community-based services
