Key Responsibilities
In your role as a Client Care Support Workers, you will actively collaborate with group home clients, families, and the care team to ensure individualized support for clients in the home
Reporting to supervisor, you will be working with adult clients and provide one-to-one care such as assisting with personal care, companionship, mentorship
Working with client to create individualized support plans with outlined goals
Ensuring the fulfillment of individual goals and needs as instructed in client support plans
Ensuring the health, safety, welfare, and independence of clients
Assisting and satisfying the social and emotional needs of a client
Documenting daily reports which may include resident incident reports, restraint monitoring, and medication administration, unusual events, or changes in condition
Adhering to the Occupational Health and Safety Regulations in Ontario
Always maintains confidentiality of client and company information
Qualifications
Secondary school certificate or equivalent
Diploma/Degree in Social Services or Healthcare or equivalent/relevant
1-4 years of experience in social service work or residential settings
Previous experience working with individuals with medical conditions and/or mental health challenges, or in a residential/live-in treatment program setting considered an asset
Strong interpersonal, communication, and computer skills
Ability to work rotating day, afternoon, overnight, and weekend shifts
Must have a valid G-class driver’s licence for a minimum of 2 years and a reliable vehicle
Current CPR and Standard First Aid Certificate considered an asset
CPI training required and considered an asset
