Responsibilities:

• Provide high-quality, patient-centered care to our residents, focusing on their unique needs and goals

• Collaborate with the interdisciplinary team to develop and implement individualized care plans

• Administer medications, perform treatments, and provide vital signs as needed

• Monitor resident progress, report changes, and make adjustments to care plans as necessary

• Engage with residents and their families to build strong relationships and promote a sense of community

• Participate in educational opportunities and training programs to stay current with industry best practices

Requirements:

• Current LPN licensure in the state of Pennsylvania

Work Schedule:

• Weekend shifts, including Saturdays and Sundays and One shift during the week.
