Job Description
Under the direction of the Manager and in partnership with local Network Leaders, the Assessor Coordinator – Degree – Senior is an integral part of operations responsible for putting strategy into action through
education
, implementation, clinical support and evaluation of the home care services provided by
interdisciplinary
care teams. They will also have a provincial role in consultation, development, and standardization of home care programming across the Saskatchewan Health Authority.
The Assessor Coordinator – Degree – Senior is the clinical and program resource for all Home Care programming needs. They will collaborate with other portfolios with the SHA to support Primary Health Care/Health Service Managers in ensuring front line staff have all the resources, training, and clinical support they require to do their work consistently, safely, effectively, aligning with current best practice standards.
They are the content expert and will have advanced knowledge of programming, policy, guidelines, standards and best practices. They will advise on, develop and implement program changes. Job Qualifications
Required Qualifications
Bachelor Degree in Nursing or Bachelor Degree in Social Work or Bachelor Degree in Physiotherapy or Bachelor Degree in Occupational Therapy Registered or eligible for registration with the appropriate licensing body Valid Class 5 driver’s license
Experience
Minimum of two years previous experience in a home care setting.
Experience using Procura and MDS-RAI assessment tools will be considered an asset
