Community Healthcare Partners is seeking a compassionate and motivated Nurse Practitioner (NP) / Advanced Practice Registered Nurse (APRN) to provide exceptional care to residents in long-term care facilities throughout the Lincoln, Omaha, and Fremont areas. Mileage reimbursement is provided.
This role offers a unique combination of primary care and wound care management, allowing you to make a significant impact on patient health while collaborating with an experienced interdisciplinary healthcare team.
Position Highlights
- Full-Time and Part-Time opportunities available
- Flexible scheduling options:
- Five 8-hour shifts /Four 10-hour shifts
Typical weekly schedule:
- 2 days in Lincoln
- 2 days in Omaha
- 1 day in Fremont
Compensation
- Part-Time Compensation: $65–$75/hour
- Full-Time Compensation:Base salary starting at $90,000
- Uncapped productivity bonus structure
- Realistic earning potential of $150,000+ annually
Facility-based patient rounding
No remote work required
Start date upon completion of credentialing
Wound Care Opportunity
- Wound care is a significant component of this position!
- Candidates must be: Currently credentialed in wound care, or willing to obtain wound care certification and credentialing
- Community Healthcare Partners will cover all associated credentialing and training costs.
Key Responsibilities
- Conduct comprehensive patient assessments and physical examinations
- Review medical histories and develop individualized treatment plans
- Order, interpret, and follow up on diagnostic testing
- Diagnose and manage acute, chronic, and infectious diseases
- Prescribe and manage medications appropriately
- Provide patient and family education focused on prevention and wellness
- Maintain accurate and timely EMR documentation (EClinical Works)
- Collaborate with interdisciplinary care team
- Deliver proactive, patient-centered care that improves quality of life
Qualifications
- Current Nebraska APRN/NP licensure
- Primary care experience with credentialing eligibility
- Wound care credentialing preferred, or willingness to obtain certification
- Strong clinical assessment and diagnostic skills
- Knowledge of healthcare regulations, compliance standards, and best practices
- Excellent communication and patient education abilities
- Compassionate, ethical, and team-oriented approach to care
- Commitment to professional development and lifelong learning
